All billing for services provided at Kincart Physical Therapy Services goes through the Davis County Hospital & Clinics.
All insurances are accepted including worker’s compensation cases and private payment options.
Direct Access is also known as self-referral. You can call and make an appointment for PT yourself, without seeing your doctor first.
Preparation for your appointment
On your initial evaluation, you will need to arrive 15-20 minutes prior to your scheduled appointment time for registration at the front desk of the Davis County Hospital & Clinics. After you register you may take the elevator to the ground floor to Kincart Physical Therapy Services. Please have a seat in the waiting room, just off the elevator.
Your first visit will last approximately 1 hour. Minors must be accompanied by a parent or guardian. After your initial visit, you may check in directly with KPTS and all other visits last between 45-60 minutes.
